Position Purpose summary:

The LPN is responsible for the assessment, planning, implementation and evaluation of resident care. The LPN provides support and direction to the Health Care Aides. The LPN is an active member of the interdisciplinary team and is accountable for providing safe and competent nursing care within the scope of LPN practice. All activities carried out by the LPN are consistent with The Salvation Army's mission and values as well as policies, procedures and protocols of Supportive Housing Program at Grace Village and Nursing department at Grace Manor respectively.

LPN working at Grace Manor must demonstrate knowledge and familiar with care of mental health clients.

The Salvation Army's Supportive Housing is a 73 bed mental health and addiction congregate living site. We are a co-ed facility. It has 24/7 Licensed Practical Nurse(s) with Health Care Aide(s) on every shift. Shifts are days, evenings, and nights. A Recreation Therapist, Resident Care Manager and Admin Support are on site during the day shift, Monday to Friday. Support is available 24/7 for nursing staff.

At Edmonton Center of Hope we strive to improve our tools and processes, so we can continue to provide excellent care and services. We use up to date technology, and an electronic charting/eMar system.

Accountabilities:

Monitoring and Assessment

  • Perform and contribute to the initial and ongoing nursing assessment of assigned Residents.
  • Collect, document and report health information from objective/subjective sources to identify Resident's health needs, monitor changes in health status using critical judgment skills, assess Resident's ability to function independently in their home environment.
  • Document timely, accurate reports of data collection, interpretation, planning, implementing and evaluating care in accordance with Salvation Army Policies & Procedures, and regulatory requirements.

Care Planning

  • Analyze information and identifies priorities, initiates and/or collaborates in the development of the plan of care to meet Resident needs.
  • Provide, initiate, and reinforce teaching using developed educational materials
  • Participate in the development of a discharge plan to meet the Resident's needs, including identifying and coordinating resources required to ensure continuity of care.
  • Identify and initiate interventions within the LPN's legislated scope of practice

Care Plan Implementation

  • Implement nursing treatments and interventions using evidence-based knowledge to accomplish plan of care.
  • Assist patients/Resident with physical requirements and activities of daily living.

Evaluation

  • Observe, report and document changes in Resident status and deviations from anticipated response/outcome.
  • Evaluate and suggest modification to plan of care in accordance with Resident care needs on an ongoing basis.
  • Make recommendations to the Health Care Team, the AHS Case Manager, Social Worker and the Manager or designate regarding changes to the treatment plan based on an ongoing assessment of the Resident's situation/status
  • Assess and monitor unstable/ill residents throughout the shift, and assumes care or consults with the Registered Nurse ; arrange transfer to acute care if necessary. Communicate promptly with AHS Case Manager and/or physician when there is a change in a resident's status

Documentation

  • Record complete and accurate relevant data for every Resident in a retrievable form according to departmental policies.

Professional Responsibilities

  • Participate in programs to develop and maintain clinical competency and promote professional growth.
  • Participate as a team member in quality improvement activities and participates in nursing practice committees.
  • Demonstrate a professional image to Residents, peers, and other health care providers and the public.
  • Serves as Team Leader providing direction and supervision to HCA care staff in a fair and equitable manner, adjusting assignments as necessary.
  • Provide nursing leadership to team members, residents and families, direct nursing care and crisis intervention as needed.
  • Responsible for reporting unskilled or unprofessional conduct, and any behavior contrary to The Salvation Army standards of conduct and policies, by any member of the interdisciplinary team to the Registered Nurse or Registered Psychiatric Nurse and Manager at the first available opportunity

Education and Research

  • Participate in research, staff development projects, in-service education classes and other activities as appropriate.
  • Participate in the orientation of new employees/students, participate in precepting students as assigned.

Occupational Health and Safety

  • Co-operate and participate fully in the Occupational Health and Safety system at The Salvation Army.
  • Identify and report hazards and incidents with the potential for injury to themselves or others to the supervisor.
  • Conduct his/her work in a safe and healthy way according to approved policy and procedure.
  • Participate in training and review provided or recommended by the employer, and take correction when not performing the work to acceptable standard.

CRITICAL RELATIONSHIP MANAGEMENT

Internal:

  • Works closely with the residents, interdisciplinary team, volunteers, family and caregivers or visitors of residents.

External:

  • Students from recognized education program, physician and outside agencies.

MANAGERIAL RESPONSIBILITY:

  • This position reports directly to the Residential Care Manager.

FINANCIAL AND MATERIALS MANAGEMENT:

  • Maintain confidential files and reports of clients.
  • Maintain a pool of resources with other community services for easy accessibility by the residents.

WORKING CONDITIONS:

  • Working environment is generally in the residential building in fast-paced conditions. The job requires lifting/twisting/bending, and will require sitting and standing for long periods of time.
  • Occasional travel within the city and some scheduled overtime is associated with this position.
  • Shift work may be required.

education and experience Qualifications:

Education, Qualifications and Certifications:

  • Graduate of an approved Licensed Practical Nurse's Program.
  • Successful completion of an accredited post-basic study program as required within the scope of the assigned position.
  • Certification as required within the scope of the assigned position.
  • Responsible to maintain and provide annual proof of active registration with CLPNA.
  • Responsible and accountable for own practice as defined by CLPNA
  • Current certification in First Aide & CPR
  • Criminal Police Investigation Check
  • Certification in Non-Violent Crisis Intervention
  • Class 5 driver's license with clean drivers abstract.

Experience and Skilled Knowledge Requirements:

  • Two years' experience in a continuing care or mental health setting preferred.

Skills and Capabilities:

  • Nursing standards/protocols as required in area of nursing specialty related to the job assignment.
  • Basic nursing care procedures.
  • Scope of practice as defined by the college.

The incumbent's work environment is typically at work sites within residential settings, and may require working under the following disagreeable conditions:

  • The incumbent will work on call 24/7 or as scheduled.

Compensation:

The target hiring range for this position is $30.62 to $33.44 with a maximum of $36.25.

Placement in the salary range will be based on factors such as market conditions, internal equity, and candidate experience, skills, and qualifications relevant to the role.
