Detailed Overview
Reporting to the Manager, Clinical Operations, the Social Worker assists residents and their families to adjust to the stress associated with chronic illness/disability, relocation, and institutional life.
Responsibilities
- Completes and updates psycho-social assessments of residents and clients; participates in pre-admission assessments, as required.
- Provides counseling services to residents/clients, families, staff and groups; make referrals to appropriate community agencies, as required.
- Attends interdisciplinary care conferences and consults with others, both internally and externally in relation to resident and client services.
- Assists with the development, implementation and review of resident and client care plans. Records actions taken on appropriate resident and client health records.
- Assists in the needs assessment, development, provision and evaluation of group work for residents/clients and families.
- Determines residents'/clients' eligibility for assistance of a financial nature. Advises residents/clients and families, and as appropriate, makes referrals.
- Participates in resident and client discharge planning.
- Attend meetings and participate in hospital and community planning, as assigned. Assists in research projects and education programs, as assigned.
- Assists in the development and provision of relevant education programs for residents/clients, families and staff Social Workers.
- Performs other related duties as required.
Qualifications
Education, Training and Experience
Bachelor's Degree in Social Work from an accredited school of Social Work. Two year's recent, related experience working as a Social Worker in a long term care setting inclusive of clinical training and experience in individual, marital/family and group modalities of social work practices.
Current full registration with the BC College of Social Workers.

- Ability to deal with residents, staff and the public in a tactful and courteous manner.
- Ability to work in a multi-disciplinary team setting.
- Ability to work with frequent interruptions and to organize and prioritize workload.
- Ability to communicate effectively both verbally and in writing.
- Must possess physical ability necessary to perform the duties of the position.
