Detailed Overview

Works as a member of the Home Health interdisciplinary team to provide assessment and treatment services for clients; liaises with community agencies and caregiver support groups; participates in educational services, planning; program development and policy recommendations; provides consultative support to the interdisciplinary team in the development of services within the assigned area of responsibility.


Responsibilities

  1. Provides therapeutic counselling services to clients and families experiencing complicated circumstances related to a variety of medical diagnoses.
  2. Conducts social work and psychosocial assessment of clients and families and interprets the information using a variety of modalities in accordance with professional standards and clinical practices.
  3. Follows up and provides ongoing monitoring of complex psychosocial situations; evaluates clients' response to interventions.
  4. As a member of the interdisciplinary team, provides information regarding clients psychosocial situation and participates in developing a comprehensive plan of care with/for the client; collaborates and consults with other health service area professionals on the care plan.
  5. Acts as a resource for clients and their families by providing information regarding community resources and access to these resources, facilitating referrals where appropriate; facilitates the development of HSA resources by identifying services needed to support the health of families.
  6. Participates in quality improvement activities by providing input into the development, implementation and evaluation of program goals, objectives, policies and procedures.
  7. Promotes client autonomy by ensuring their participation in the planning, decision making and the development of problem solving abilities.
  8. Assists in the development and provision of relevant educational programs for home health care staff, clients, their families and community agencies on psychosocial issues related to health, illness and disability.
  9. Prepares statistics, in accordance with the workload measurement system, for submission to designated personnel; maintains written clinical records as required.
  10. Performs other related duties as assigned.

Qualifications

Education and Experience

Master's Degree in Social Work from an approved school of Social Work. Two (2) years' recent related experience in a complex health care environment such as a community or home health setting, or an equivalent combination of social work education, training and experience.

Current full registration with the BC College of Social Workers. Valid BC Driver's License and access to personal vehicle for business-related purposes.

  • Comprehensive knowledge and understanding of counselling skills and clinical practice models as it relates to individuals, families and groups
  • Demonstrated ability to conduct social and psychosocial assessments
  • Ability to provide leadership to the interdisciplinary team in care planning in complex client situations
  • Ability to work effectively with others including working effectively in a multicultural environment
  • Ability to work collaboratively as part of an interdisciplinary team and understanding the scope of practice of other health care disciplines
  • Ability to make effective referrals, reflecting in-depth knowledge of community resources and strong collaborative skills in working with other community agencies
  • Demonstrated ability to communicate effectively both verbally and in writing with clients, families, other members of the health care team and community agencies
  • Demonstrated ability to effectively manage conflict
  • Ability to facilitate group meetings, processes and decision making
  • Ability to operate related equipment including related computer software
  • Physical ability to perform the duties of the position
