POSITION SUMMARY:

Background on Our Dementia Journey Journal

Our Dementia Journey Journal (ODJJ) is an interactive resource that is designed to enable sustainable relationship-building between caregivers and care providers of persons living with dementia through the sharing of meaningful and relevant information pertaining to physical, functional and social care and support. It also aims to help caregivers and care providers (re)negotiate their respective roles as the circle and/or setting of care changes along the dementia journey and optimize their respective knowledge of and about the persons living with dementia PLWD). The evidence-informed ODJJ is available on analog, digital and mobile platforms and is available in English, French, Hindi, and Punjabi. From spring 2024 - fall 2024, the ODJJ has been implemented and evaluated in LTC homes, First Nation communities and South Asian communities. In the next phase of the project, the team is looking to implement the ODJJ in additional communities across Canada.

We are currently seeking a passionate and dynamic Temporary Full Time Community Engagement Lead to support the implementation and rollout of the Our Dementia Journey Journal (ODJJ) across various organizations such as Alzheimer's Societies, adult day programs, long-term care homes, retirement homes, and other community settings across Canada. In this role, the successful candidate will build and nurture relationships with key community partners in these organizations to introduce and facilitate the adoption of the ODJJ resource. They will be tasked with executing an implementation plan with marketing materials and supporting the resource's integration into diverse community settings. This is a 4-month contract position.

The Community Engagement Lead works closely with:

  1. the ODJJ Project Lead to support and lead implementation efforts in different communities,
  2. the ODJJ Evaluation Lead to learn of past patterns of uptake, use and impact from the evaluation
  3. the cross-Canada ODJJ Project Advisory Committee to gain insights on the implementation of the ODJJ across multiple communities, and
  4. the ODJJ Marketing Lead to supporting building the marketing strategy and campaign for the ODJJ for Canadian users by one or more organizations into the future.

POSITION RESPONSIBILITIES:

  • Community Outreach & Engagement:
    • Develop and maintain relationships with key community partners in each organization to facilitate the introduction and adoption of the ODJJ.
    • Work with organizations and community partners to develop implementation plans and strategies for their settings
    • Provide training and onboarding support to organizations on how to effectively use the ODJJ, ensuring that all relevant partners (caregivers, healthcare providers, family members) are well-informed.
    • Work closely with community organizations to gather feedback, ensuring the ODJJ is effectively integrated and continuously improved based on user experiences.
    • Track and report on the progress of the rollout in different organizations, maintaining accurate records of interactions, challenges, and successes.
    • Manage and lead the pan-Canadian Project Advisory Committee: Organize and facilitate monthly (virtual) meetings of this multidisciplinary group of experts-by-experience to guide the implementation of the ODJJ
  • Marketing & Promotion
    • Roll out early stages of the marketing plan for the ODJJ, targeting additional organizations such as the Alzheimer's Society, adult day programs, long-term care, retirement homes and additional community organizations
    • Organize and facilitate community events, presentations, and meetings to build awareness of ODJJ
    • Market the ODJJ resource to new communities, emphasizing its value in supporting dementia care and fostering stronger relationships within the Circle of Care.

QUALIFICATIONS

  • Education:
    • Bachelor's degree in related field (e.g. Healthcare Administration, Marketing, Communications, Community Engagement, Social Work, etc.)
    • Additional certifications or training in dementia care, community outreach, or marketing would be considered an asset.
  • Experience:
    • Minimum 3-5 years' post-graduation experience working in community outreach/engagement, event coordination, etc. preferably within the healthcare or social services sectors.
    • Experience in rolling out programs or resources to various community-based organizations is highly desirable.
  • Skills and Abilities:
    • Strong communication skills, both written and verbal, with the ability to present complex information in an easy-to-understand way.
    • Proven ability to build and maintain relationships with diverse community partners, including healthcare providers, community organizations, and families.
    • Ability to work independently, manage multiple priorities, and meet deadlines in a fast-paced environment.
    • Knowledge of Alzheimer's disease and related dementias and their impact on the person, care partner, family and health care providers is considered an asset
    • Excellent organizational skills and attention to detail.
    • Excellent time management skills
    • Comfortable with digital tools and resources, including email marketing platforms, and social media.
