What we are looking for:

Working as an integral member of the inter-professional team, you will play a critical role in providing social work services to patients and their families within the organization, in the acute care setting, with the exception of in-patient mental health. There is a strong emphasis on psychosocial support, interdisciplinary problem solving, and liaising with appropriate members of the team to ensure patients and families are linked with necessary resources and supports. Within a fast paced environment you will be able to prioritize a very diverse caseload of patients and respond to crisis situations as they arise

The successful candidate will demonstrate exceptional leadership skills, a high level of professional integrity and accountability and play a key role in system development, as well as possess a strong level of understanding of care across the continuum. Experience in short-term supportive counseling with patients and families dealing with difficult decisions and end of life concerns would be a valuable asset.

Duties and Responsibilities will include:

  • Completing psychosocial assessments
  • Short-term, goal oriented support and counseling for patients and families dealing with grief, chronic life altering issues as well as end of life concerns
  • Working with the health care team, patient and family to develop patient care plans
  • Cross coverage for other Social Workers within the hospital
  • Consultation with community health care professionals
  • Maintaining up dated knowledge of community resources
  • Attending and responding to gaps in service delivery
  • Attendance on committees and tasks forces as appropriate

What you bring to the role:

  • Master of Social Work
  • Registration with the Ontario College of Certified Social Workers and Social Services Workers
  • Recent related previous social work experience in a hospital environment
  • Understanding of loss/adjustment and the grief process
  • Demonstrated strengths in interpersonal relations, problem solving, and conflict resolution
  • Demonstrated excellent communication skills
  • Demonstrated commitment to professional development
  • Experience in working with electronic documentation, preferably, Meditech
  • Demonstrated good attendance and performance records with the ability to maintain these same standards
  • Demonstrates a commitment to a healthy and safe workplace for self and others (staff, patients, families, etc.) by taking all reasonable precautions and working in compliance with hospital related policies, health and safety legislation and best practices and completing relevant mandatory education as required

Work Schedule:

The successful candidate will be available to work between the days of Monday - Sunday in 8 hour day shifts, primarily between 08:30 - 16:30.

It is the responsibility of casual staff to provide reasonable availability to their department, inclusive of a minimum of two weekdays and two weekends within each 6-week period. The successful candidate will also rotate to cover the Uxbridge site remotely.

OPCOM13: $44.80 - $57.08 per hour
