We are currently hiring Part-Time Personal Support Workers (PSW) for Community Care in our Assisted Living Facility/for Community Care

Requirements:

QUALIFICATIONS:

  • Personal Support Worker certificate
  • Current Vulnerable Sector Police Check
  • Must be able to work scheduled shifts as well as provide back-up to others
  • Ability to work independently without direct supervision
  • Ability to work in and promote a positive and supportive team environment
  • Demonstrated exemplary communication skills both verbal and written
  • Computer literacy is a requisite
  • Demonstrated leadership experience

RESPONSIBILITIES:

  • Performs skills as per standards for Personal Support Worker (PSW)
  • Carries the main PSW phone when on shift, follows up on all calls and emergencies and PSW team and updates regularly via computer
  • Effectively leads agency staff in the provision of client care as per client care plan and under the direction of the SSC and SE RPN Manager as appropriate
  • Shadow and train new staff, share updates at shift change huddles
  • Ensure a key count is completed at each shift
  • Storage of reports, education information, communications
  • Print the necessary documents needed for client files, 24-hour report, keep master copies up to date
  • Appropriate and timely communications with SE RPN Manager/SSC/PSW's regarding client issues/concerns
  • Appropriate and timely communications with clients and family members
  • Appropriate and timely communications with supervisor regarding staff performance issues

File management:

  • Document and communicate in clients' health records according to agency and Halton Region policy and ensure staff team is documenting and maintaining files
  • Record new and appropriate client information in the communication book, i.e., clients returning home from hospital, PRN needed, etc.
  • Collaborate with SSC for file maintenance and storage of closed files and yearly file thinning
  • Ensure staff are shredding confidential client information that is copied and used during each shift, such as client task schedules
  • Help to keep Emergency Binder up to date. Assists agency scheduler with staff scheduling, book off, and sick calls
  • Problem-solves with clients and families
