Ontario Health atHome is looking for an experienced Senior Leader who contributes to the overall success of Ontario Health atHome in the West Region by providing effective leadership and strategic direction in patient care and contract management to Hamilton, Niagara, Brant, Burlington and Erie St. Clair region.
Reporting to the Vice President, Contracts, the Director, Patient Care Contract Management is responsible for managing the overall Quality and Performance Management process related to Ontario Health atHome Providers and Vendors. This position oversees all aspects of Patient Care Service Provider Organization (SPO) and Vendor contracts, including the negotiation, preparation, monitoring and evaluation in accordance with performance standards and ministry-defined goals.
