Are you passionate about quality assurance in healthcare? Unity Health Toronto is seeking a Nurse Practitioner to join our Houses of Providence team. Reporting to the Director of Care and working closely with the Medical Director and all attending Physicians and specialists within an inter-disciplinary team, the Nurse Practitioner responds to the specialized needs of the residents. At HOP, NPs provides care to residents within the legislated scope of practice for NPs, as described in a collaborative practice agreement among the Hospital, collaborating Physician partners and the NP; provide any care that is beyond the legislated scope of practice as defined within authorizing mechanisms, such as medical directives and delegation. Other duties include:

Duties & Responsibilities:

  • Works autonomously to diagnose, order and interpret diagnostic tests, prescribe pharmaceuticals and perform procedures within the legislated scope of practice and defined collaborative practice agreement, to provide care to a defined population and demonstrates the following competencies: Advanced Health Assessment and Diagnosis; Therapeutic Management; Health Promotion and Prevention of Illness and Injury.
  • Collaborates with Physician partners and the inter-professional team, to provide care to residents that reflect shared goals, and features enhanced communication, compassion and continuity. Many NP roles provide integrated and comprehensive care to residents, while others provide consultative care. The NP explicitly consults Physicians and the inter-professional teams for care concerns that are not within the scope of practice.
  • Participate in identifying, analyzing and interpreting trends in resident care and professional nursing practice to determine priorities for educational programming for team members, residents and families.
  • Participate in Quality Committee meetings; lead and support CQI projects and initiatives and participate in the implementation of Clinical Quality Improvement Plans.
  • Attend and/or support internal and external committees as required.
  • Ensures competencies and clinical practice are within standards, practice guidelines, and policies of the Ministry of Long-Term Care, HOP and the College of Nurses Standards.
  • In keeping with professional standards of practice, obtains and/or disseminates information, which is relevant to meeting the residents' identified needs and with appropriate authorization, either in the form of signed consents or direct authorization from the resident or designated substitute decision-maker.
  • Attends resident safety in-services and takes action to address unsafe conditions and procedures.
  • Ensures fully knowledgeable of accountabilities for the health and safety of all workers, volunteers and contractors under supervision. Ensures compliance with established safe work practices, policies and procedures, and conducts thorough investigations of all workplace incidents


Qualification:

  • Minimum one-year clinical and/or working experience in geriatric, rehabilitation, or long-term care nursing.
  • Leadership knowledge and/or experience are considered an asset.
  • Certifications obtained or in progress that includes gerontology, mental health, behavioral support and palliative care is an asset.
  • Current registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class (Primary Health Care or Adult) and entitled to practice.
  • Working Knowledge of the; Long Term Care Act, Program Standards for Long Term Care, Health Disciplines Act, Occupational Health & Safety Act, Pharmacy Act, Food and Drugs Act (Canada), Narcotic Control Act (Canada) and Coroners Act, is considered an asset Must be available to be on site.
