Job Description

Department: PMCC Outpatient Cardiac Clinics
Work Model: On-Site
Reports to: Nurse Manager
Hours: 37.5 hours per week
Salary: To commensurate with years of experience
Shifts: 5 shifts per week; Days 8.00am - 4.00pm
Status: Temporary Full Time (October 2025)
Closing Date: March 31, 2025

Position Summary
The Peter Munk Ambulatory Cardiac Center provides resources designed to improve the healthcare of patients with new and on-going cardiac conditions. The Nursing Clinical Coordinator in the PMCC ACC, in collaboration with the healthcare team, will facilitate patient care, improve the education of the patient and provide in the moment interventions as ordered. The Registered Nurse is responsible and accountable for providing direct and indirect patient care in the Ambulatory Cardiac Clinic.

Duties

As a member of the Ambulatory Clinic team, the responsibilities of the Clinical Coordinator, include:

  • Obtains/reviews/triages medical records
  • Vital signs
  • IV insertion and management of IV
  • Administration of clinic approved IV medications
  • Recognition of potentially harmful arrhythmias with knowledge and ability to obtain immediate medical interpretation and direction and provides education to our ambulatory population
  • Utilizing and applying current specialized knowledge and expertise on recent advances in nursing practice to facilitate the effective management of clinical activities
  • Coordination of patient care across the continuum and continuity in patient care management
  • Performing patient care responsibilities; performing teaching/educational responsibilities
  • Participating actively in Quality Improvement initiatives
  • Participating in academic and research activities/projects
  • Priaging general cardiology patients and Adult Congenital patients

Qualifications

  • Current registration with the College of Nurses of Ontario
  • Bachelor of Science in Nursing required
  • Current BCLS Certification
  • Minimum five (5) years Cardiac +/or Cardiac Surgery experience required
  • Critical Care or Coronary Care Certification preferred
  • Ambulatory experience preferred
  • PICC line access, maintenance and removal
  • Experience working with community health providers preferred
  • Experience in change management
  • Strong organizational and time management skills
  • Excellent decision-making and problem- solving skills
  • Excellent interpersonal and customer service skills
  • Excellent verbal and written communication skills
  • Demonstrated ability to cultivate and build relationships
  • Strong track record of high- quality work and engagement is essential
  • Able to adapt to a constantly changing environment that requires ongoing revaluation of priorities
  • Demonstrated ability to use good judgment in assessing difficult situations
  • Able to work collaboratively within a group setting and independently - required
  • Proven track record of taking initiative
  • Computer literacy required - Word Suite
