Job Description

Reports to: Senior Clinical Manager
Hours: 37.5 hours per week
Salary: $39.17 - $48.96 per hour
Shifts: Days
Status: Temporary Full-Time ( 12-18 months )
Closing Date: February 26, 2025

As a member of the multi-disciplinary Eating Disorders Program at the Centre for Mental Health at UHN, the Clinical Intake Coordinator facilitates the timely access to clinical assessment and treatment for patients referred to the program. Through collaboration, strong communication, sound clinical judgement, and excellent administrative organizational skills, the Coordinator plays an essential role in patient admissions to both the inpatient and outpatient units of the Program.

Duties

  • Liaise with patients and referral sources, facilitating patients' admissions to treatment pathways
  • Assign patients to Clinician caseloads
  • Monitor program capacity, working to maintain optimal volumes in line with program goals and key performance indicators
  • Work as part of a multi-disciplinary team to support timely patient admissions to the Eating Disorders inpatient and outpatient units​​​
  • Identify gaps in admission flow and actively pursue solutions
  • Communicate with team members regularly regarding admission activities
  • Collaborate with program leadership to plan, develop, implement, and evaluate admission process improvements
  • Participate in quality improvement initiatives

Qualifications

  • At minimum, completion of a bachelor's degree in Health Sciences, although a Master's Degree is preferred
  • Registration/licensure as required by professional body, e.g. Ontario College of Social Workers and Social Service Workers, College of Occupational Therapists of Ontario, etc.
  • Experience working with patients with Eating Disorders an asset
  • Exercise initiative and sound judgement
  • Experience with cognitive behavioural therapy an asset
  • Experience working in a hospital environment an asset
  • Excellent interpersonal skills
  • Excellent administrative organizational and time management skills
  • Ability to respond to emerging urgent issues related to admissions and referrals
  • Ability to independently manage multiple priorities and competing demands
  • Ability to work effectively independently and as a member of a multidisciplinary team
