Job Description

The M. Lau Breast Centre at Princess Margaret is the largest comprehensive breast cancer centre in Canada. We offer cutting-edge, novel programs in screening, diagnosis and assessment, treatment planning and delivery, as well as follow-up care, patient education and survivorship support. Our inter-professional team of medical, radiation and surgical oncologists works in collaboration with breast imaging specialists, breast-specific pathologists, plastic and reconstructive surgeons, nurses, social workers, dietitians and other allied health professionals. Our centre offers patient-centred care that treats the whole person, not just the disease.

Duties

  • Design, implement, and evaluate a nurse-led model for oversight of treatment plans of breast cancer patients with CDK4/6 inhibitors in ambulatory care.
  • Establish and optimize the patient pathway for existing breast cancer patients with CDK4/6 inhibitors, ensuring seamless transition and continuity of care.
  • Provide ongoing follow-up care in-person and virtually to assess side effects of CDK4/6 inhibitors.
  • Act as the primary point of contact for patients.
  • Provide quality health care through patient education and counseling on treatments, appointment expectations, treatment plans, patient navigation, and team communication.
  • Manage referrals within existing nursing scope of practice and medical directives across oncology disciplines within the breast site group, as well as partners (e.g. homecare, psychosocial oncology).
  • Collaborate with the multidisciplinary team to address and advocate patient concerns.
  • Facilitate integration of therapies into clinical practice by helping develop protocols including development of new medical directives.
  • Establish processes to assess and present patient-reported outcomes, including the development and implementation of remote patient monitoring programs to support continuous care and proactive intervention.
  • Develop patient care educational materials for breast cancer patients with CDK4/6 inhibitors.
  • Lead quality improvement initiatives to enhance patient care and program efficiency.
  • Provide educational sessions and contribute to the professional growth of the breast care team.

Qualifications

  • Current certificate of registration from the College of Nurses of Ontario.
  • Completion of a Graduate Degree Program in nursing or a recognized equivalent.
  • Current certification in Basic Cardiac Life Support (required).
  • 3-5 years of experience in oncology nursing, preferably with contemporary experience in symptom management of breast medical oncology patients.
  • Certification in Oncology Nursing (preferred).
  • Membership in relevant professional organizations preferred (e.g., RNAO, CANO).
  • Program planning, development, implementation, and evaluation methodology skills.
  • Experience with clinical research and data collection methodologies.
  • Eligibility for cross-appointment to an academic institution.
  • Knowledge of patient-reported outcomes and quality improvement methodologies.
  • Exceptional communication, organizational, and time management skills.
  • Proficiency in utilizing information technology and data management tools.
  • Excellent coaching and mentoring skills.
