The Toronto Grace Health Centre (TGHC) provides specialized care and services to those individuals who require Complex Continuing Care, Post Acute Care Rehabilitation, Palliative Care, and Transitional Care. The care and services provided at TGHC help facilitate the patient flow through the health care system and ensures recovery to those individuals who may return to their homes or to Long Term Care (LTC) facilities in their communities.

HEALTH RECORDS ASSISTANT

POSITION SUMMARY
Reporting to the Health Records Manager/Privacy Officer, the Health Records Assistant will ensure that patient charts are maintained in accordance to the standards, vision and values of the Health Centre, professional practice and legislative requirements. The Assistant will process health records while safeguarding patient privacy and confidentiality at all times.
QUALIFICATIONS
•Graduate of an approved Health Information Management program or student in Health Information Management
•Must be an active member of the Canadian Health Information Management Association
•Minimum one (1) year experience in Health Records Department or related job experience
•Strong organizational skills,
•High degree of accuracy and attention to detail.
•Ability to communicate effectively both verbally and in writing
•Clerical and proficient computer experience
•Knowledge of PHIPA and FIPPA
•Ability to work independently and as a member of a team
•Excellent attendance record.

MAIN RESPONSIBILTIES
1.Management of Patient Information /Charts
• Assemble patient chart and check records for deficiencies
•Track, pull and file patient charts
•Review, distribute and archive transcribed reports
•Sort and file loose reports. Scan paper documents.
•Review chart documentation and verify demographic and admission details and update accordingly
•Prepare statistical reports on excel
2.Provides coverage to other Health Information Clerical employees as required
