Job Description

Work Model: On-Site
Hours: 22.5 hours per week
Salary: $50.04 - $62.55 per hour
Shifts: Days ONLY (no nights or weekend shifts)
Status: Temporary Part-Time (1-2 year contract - with an option for permanent PT)

The Jay and Sari Sonshine Centre OUTPATIENT NURSE PRACTITIONER in Lifestyle and Health Promotion demonstrates autonomy while assuming and having sole accountability and the authority to independently communicate a diagnosis, prescribe drugs and order any necessary diagnostic tests, the principal responsibilities include: utilizing and demonstrating advanced knowledge and decision making skills to conduct comprehensive assess, diagnose and manage the care of patients; planning and implementing strategies to promote health and prevent illness with individuals, families and groups; delivering, coordinating and participating in educational and professional development activities/programs.

This position is dedicated to enhancing the quality of education and the prevention of noncommunicable diseases. It aims to support stroke prevention and optimize brain care by utilizing the model of care developed by our program, and in focused on lifestyle medicine pillars.

The JS Sonshine Centre was officially established in October of 2023 with a mission to bring patients, communities, clinicians and scientists together to work on preventing stroke and preserving brain health through better brain care. The centre is founded on principles of the lifestyle medicine paradigm to provide enhanced quality of outpatient services tailored to patient-directed care for: better prevention of future stroke, improved quality of life post-stroke/TIA, and optimized brain care over years for all.

The three overarching pillars of the JS Sonshine Centre are: a) Innovation to catalyze lifestyle medicine integration into mainstream clinical care; 2) Education in order to optimize promotion of brain health and stroke prevention; and 3) Research collaborations in order to advance brain health care both nationally and internationally.

Duties

  • Utilizing and demonstrating advanced knowledge and decision-making skills to conduct comprehensive assessments within the scope of this position
  • Diagnose and manage the care of patients with stroke or at risk for stroke.
  • Planning and implementing strategies to promote health and prevention of stroke
  • Prevent illness (e.g., hypertension, diabetes) with individuals, families and groups.
  • Delivering, coordinating and participating in educational and professional development activities/programs.

HEALTH PROMOTION & EDUCATION

  • Plans and implements strategies to promote health and prevent illness with individuals, families and groups, and manages specific health and lifestyle problems (supports health behavior changes, challenges and facilitators)
  • Initiates individual, family and group promotion of health/wellness and group teaching (group visits)
  • Identifies learning needs of various populations (based on social determinants of health)

COMMUNITY PARTNERSHIPS

  • Works with representatives of community agencies to plan and deliver group-based health promotion, disease prevention and education activities.
  • Works with community agencies to plan interventions relating to community assessment, development and program planning.
  • Collaborate with the Patient Education and Engagement Leader to introduce innovations into primary care settings and community centers.
  • Plays an active role in advocacy and shaping public policy in health care, including working with appropriate community representatives in support of health policy development and practice.
  • Develops and maintains effective working relationships with existing community and professional groups, including acting as a resource and serving as a consultant to individuals and groups within the professional community and other hospital/agencies.

Qualifications

  • Completion of master's degree Program in Nursing ( or one semester from completion)
  • Registered Nurse (EC) certification complete or pending
  • Nurse Practitioner (NP) Adult or Primary Care certificate complete or pending
  • Current Certificate of Competence with College of Nurses of Ontario
  • Mid-career professional expertise with over at least 5 -7 years experience in Primary and/or Community Care and Health Promotion - working with NCD spectrum
  • Experience in nursing research and quality improvement methodology
  • Ability to perform duties in a professional and courteous manner and produce high quality work while meeting deadlines in accordance with UHN standards
  • Exercise initiative and good judgement with ability to multi-task
  • Compliance with confidentiality requirements
  • Effective organizational, interpersonal and communication skills
  • Knowledge of applicable legislative, UHN and/or departmental policies
  • Client service oriented, with the ability to effectively work with diversity and appreciate that people with different opinions, backgrounds and characteristics bring richness to the challenge or situation at hand
  • Evidence of strong assessment skills and the ability to predict changes in patient status
  • Demonstrated knowledge and ability to utilize process of planning, implementing and evaluating patient care
  • Demonstrated leadership/in charge skills and evidence of good organizational and decision-making skills
  • Evidence of commitment to team building and working within a patient focused care model
  • Program planning and evaluation methodology skills
  • Commitment to collaborative practice
  • Ability to utilize information technology
  • Experience in a triage role in an emergency department setting
