Position Summary: Our center is seeking a dedicated and dynamic individual to join our multidisciplinary team as a Patient Education and Engagement Leader in Community- Based Brain Health Promotion. This role will focus on employing evidence-based knowledge, patient-lived experiences, and community needs to combat age-related brain diseases - specifically stroke prevention.

The successful candidate will engage in community outreach and professional activities, such as educational programming and brain health promotion, but also quality improvement, both locally and globally, aiming to make a significant impact on stroke and cerebrovascular disease prevention and care. The role is focused on engagement of diverse population (e.g., women, low resource population)

The JS Sonshine Centre was officially established in October of 2023 with a mission to bring patients, communities, clinicians and scientists together to work on preventing stroke and preserving brain health through better brain care. The centre is founded on principles of the lifestyle medicine paradigm to provide enhanced quality of outpatient services tailored to patient-directed care for: better prevention of future stroke, improved quality of life post-stroke/TIA, and optimized brain care over years for all. The three overarching pillars of the JS Sonshine Centre are: a) innovation to catalyze integration of clinical care; 2) education to optimize promotion of brain health; and 3) research collaborations to advance brain health both nationally and internationally.

Duties

  • Foster relationships with community leaders, local community partners and organizations for sites to support implementation of program-related work and projects.
  • Prepare materials, consent forms, and interview guides.
  • Support participant recruitment, obtain consent.
  • Create data collection tools in REDCap
  • Coordinate data collection (e.g., surveys, focus groups, one-on-one interviews).
  • Support data analysis (e.g., quantitative and qualitative).
  • Organize community workshops and seminars on brain health and stroke/dementia prevention.
  • Develop and distribute educational materials that are tailored to the respective community and population.
  • Prepare and submit ethics and quality improvement-related applications.
  • Develop and share monthly updates and knowledge mobilization products.
  • Assist in preparing manuscripts for publication and grants for additional funding
  • Prepare and present findings at conferences.

Qualifications

  • Master's degree or higher in a health-related field or equivalent combination of education/experience
  • At least 4 years practical and related experience
  • Knowledge and experience of adult education theories and practice, adult education, the determinants of health as demonstrated through a combination of education and practical experience
  • Knowledge of principles of effective health communication, health literacy, plain language, patient engagement
  • Prior experience in community engagement and grant writing
  • Outstanding communication (written and verbal), customer service, and listening skills that demonstrate compassion, empathy and respect for colleagues, patients and families
  • Effective organizational and interpersonal skills including time management, decision making and problem solving
  • Excellent teaching skills in formal and informal settings with the ability to teach one to one, small and large groups
