As a Physiotherapist, you will assess and determine patient care needs and eligibility, provide access and referrals to community services, and engage with patients, caregivers and other health care practitioners.

The Physiotherapist plans, organizes and directs diverse programs of treatment, counseling, screening and education for ONTARIO HEALTH ATHOME Champlain Patients in a variety of settings (in homes and retirement homes)

Occasional shifts : Monday to Friday – 8:30am to 4:30pm
We offer flexible work options, this position will have a hybrid model of work with a combination of in-office and telework.
Location: Can be one of: 709 Cotton Mill St, Cornwall; 530 Fred St, Winchester; 4200 Labelle St, Ottawa or 119 Main St E, Hawkesbury
Assignment: Will cover clients primarily in Eastern Counties
