• Bachelor of Social Work Degree or Social Service Worker certificate and membership in good standing with the Ontario College of Social Workers and Social Service Workers is required
• A minimum of 5 years’ experience in Seniors’ services and programs in the community, Retirement Homes, Long Term Care or Hospital settings
• Ability to review, assess, consult and intervene utilizing non-pharmacological methods with clients who are experiencing behavioural difficulties and have been referred to the Behavioural Supports Ontario mobile team.
• GPA, PIECES, U-First, DementiAbility, ASIST certification an asset
• Understanding and effective utilization of non-pharmacological approach to care when managing dementia patients & supporting families/caregivers.
• Analytical and organizational skills with a demonstrated ability to handle and respect confidential information with tact and diplomacy.
• Excellent understanding of the geriatric/elderly population sector and a further understanding of patients/families experiencing mental health concerns in current health care climate.
• A minimum of 5 years’ experience working in the seniors’ services and programs within the community, Long Term Care facilities or Hospital settings (complex continuing care etc.); Mental Health and Addictions services and settings.
• Ability to work independently managing clinical caseload volumes; including new referrals/intakes, an ability to provide short-term, goal specific case-management, and provide discharge & transitions supports.
• Ability to work collaboratively while demonstrating active participation with community partnerships (existing & new).
• Effective time & resource management.
• Participate in broader care-planning initiatives and strategies for patients across the continuum of care.
• Knowledge & understanding of roles & responsibilities within the team & departmentally and the organization.
• Strong knowledge, or willingness to learn regarding the available resources within the Oxford County Community available for older adults and their families.
• Understanding care transition pathways for older adults/families within the current healthcare climate and knowledge of the LTC application process.
• Delivery of services in the community, Long term Care settings as well as in hospitals, retirement homes, offices and clients homes in Oxford County.
• Facilitation of monthly presentations to professions and community groups on topics related to the BSO model.
• Ability to establish and maintain positive relationships.
• Excellent communication skills – both written and verbal (ability to provide appropriate feedback regarding progress of clients).
• Participation in clinical and administrative meetings necessary for the effective performance in the BSO Mobile Team program.
• Ability to work in a fast-paced multidisciplinary hospital and community team environment.
