RESPONSIBILITIES

• Review, screen and determine acuity and eligibility for persons referred to Mental Health Services, including but not limited to General Outpatient Psychiatry, Speciality Psychiatry Clinics, the outpatient Eating Disorder Program, and the PEPP-First Episode Psychoses Program, in addition to internal referrals made within the department. The level of involvement the Intake Clinician has with referrals is dependent on the program to which the person was referred to. Involvement may include conducting a full biopsychosocial assessment, liaising with the referral sources, collecting collateral information, and providing education to the person/family involved with direct care.
• The position, alongside the Intake Coordinator, also maintains any waitlists for outpatient programs and is required to be a contact/resource person for patients on the waitlist for various treatment teams (e.g. counselling waitlist, eating disorder treatment, etc.).
• Attend weekly Outpatient Team Rounds for the purpose of distributing any pending referrals to outpatient teams that are accepting referrals and/or presenting acute referrals requiring immediate service.
• On a weekly basis, the Intake Clinician, alongside the Intake Coordinator, is responsible for coordinating the Talk-In Counselling Clinic. This includes running the clinic on the scheduled date, conducting/screening pre-interview paperwork, assigning patient files to therapists, liaising with registration/departmental clerical staff, provide peer supervision with therapists during the clinic., and maintain weekly, monthly, and annual stats for the Talk-In Counselling program. Tasks may alter during the operation of virtual clinics during pandemic responses.
• The Intake Clinician may also be required to provide individual therapy sessions in the Talk-In and Brief therapy programs based on coverage needs.
• Develop and maintain constructive working relationships with community agencies, primary care, and other professionals having contact with Mental Health Services. The clinician will encourage positive public relations, co-ordination of services and information sharing, and also participate on hospital and community committees.
• Receive regularly scheduled clinical and administrative supervision by the Director of Mental Health Services or delegate.
• Provide electronic assessments, complete progress notes and termination summaries for patient files, as needed.
• Participate in case conferences, team meetings, Mental Health departmental meetings, to ensure the co-ordination and planning of patient programs and services with the centralized intake service.
• Assist patients with referrals through establishment of liaison with community agencies. Identify resources that may become necessary in providing for the fulfillment of patient directed needs and assist with discharge planning (may include assistance with forms, applications, etc.)
• Participate in clinical and administrative unit meetings, social work meetings, and other meetings necessary for the effective performance as a social worker within the hospital.
• Keep abreast of new developments within the field of social work and related fields. Maintain and upgrade skills as required.
• Act as Field Practicum instructor to Social Work students placed at the hospital, as requested by the Director of Mental Health Services.
• Carry out a teaching role within Mental Health Services through participation, and presentation at the regularly scheduled staff education seminars and other educational forums existing within the hospital. This role may also extend to the community.
• Maintain timely and accurate statistical records of activities for Departmental, Hospital, and Ministry purposes.
• Consult with Director of Mental Health Services, when administrative, therapeutic, or legal matters could result in significant or serious outcome to patient, hospital, or therapist.
• Carry out other duties as required by the Director of Mental Health Services.

JOB REQUIREMENTS

• Excellent biopsychosocial assessment, formulation, and treatment skills
• Knowledgeable in the modalities of solution-focused therapy, CBT, DBT and Motivational Interviewing
• Superior interpersonal skills
• Superior written and verbal communication skills are mandatory
• Demonstrated ability to deal congenially with a variety of people efficiently and effectively with minimal supervision
• Organizational skill and demonstrated ability to handle and respect confidential information with fact and diplomacy are essential.

EXPERIENCE
• Minimum of 3 years of recent mental health experience

REGISTRATIONS
• Must be a member in good standing with the Ontario College of Social Workers and Social Service Workers
